For the application process you must come into the main office at Westbrooks Towers. You must bring in with you the items in the following list.

• Social security card
• Medicare card
• Medical Insurance card (Must have name and address of medical insurance company)
• Drivers license or a picture ID
• Birth certificate (if possible)
• Proof of income (If you have a spouse, bring in their proof of income as well)
• Name, address, and account numbers of all bank accounts
• Name and addresses of all associated pharmacies
• Name and addresses of all associated doctors
• Pension papers, which include name and address
• Pension papers on spouse, which include name and address
• If you have property, bring in your county tax card.
